Care Talk magazine are delighted to be the organisers of the Palliative and End of Life Care Awards, recognising and celebrate excellence in palliative and end of life care in a social care setting, hospice or community hospital.
This unique event recognises initiatives within social care, led by social care professionals, that improve how people live and die towards the end of their life. The awards will shine a light on the complex role social care plays in this area, which often encompasses the care of people living with dementia, complex physical and neurological disorders, as well as other life threatening conditions.
The aim of the awards is to pay tribute to individuals and organisations who provide high-quality, person centred, end of life and palliative care – enabling people to have a Good Life and a Good Death; recognising accomplishments at all levels from within the social care workforce. These include services for older people, people with learning disabilities and/or autism, volunteers and suppliers of innovative products and services.
There are a variety of categories available for nomination that represents all areas of palliative and end of life care in a social care, hospice or community setting – all with the common thread of celebrating outstanding excellence, best practice, dedication and innovation within this domain.
