This award celebrates home-based social care for people living and dying at home. Recognising innovative, quality driven, person centred domiciliary care which makes a real difference to the wellbeing and comfort to individuals in their end of life journey.  A care provider that has a culture of wellbeing, person centred care, workforce development and community involvement.  A care provider who works in partnership with hospices through training, innovation and enabling seamless continuing care.

The Palliative Nursing Home Provider Award

This award celebrates care for people living and dying in later old age. Recognising an innovative, quality driven, person centred nursing home provider  that makes a real difference to the wellbeing and comfort of individuals in their end of life journey.  A nursing home that has a culture of wellbeing, person centred care, workforce development and community involvement and who works in partnership with hospices through training, innovation and enabling seamless continuing care.

The Palliative Care Manager Award

This award recognises a Manager of end of life care in a hospice, residential/nursing home or community setting.   A manager who demonstrates exceptional leadership skills and an ability to create a positive and supportive work environment for staff.  An ability to effectively manage resources, ensure compliance with regulations and standards, implement quality improvement and demonstrate a commitment to enhancing the overall patient experience, should also be evident.

The Ancillary Worker Award

This award recognises an individual’s dedication to providing exceptional support services, an ability to promote wellbeing and create a comfortable and welcoming environment for people in receipt of care. Professionalism, teamwork and a commitment to ensuring the smooth operation of ancillary services and/or meaningful activities that contribute to holistic care in a social care or hospice setting must also be evident.  Nominations for all ancillary staff including activity organisers are welcome.

The Support for Family Carers Award

This award recognises teams and individuals who go above and beyond in their dedication to providing exceptional support for family carers.  Promoting wellbeing for people living and dying towards the end of their lives and an ability to create a comfortable and welcoming environment for people in receipt of care and their families should be evident.

The Innovation in End of Life Social Care Award

This award recognises outstanding innovative approach(es) that have a positive impact on the person receiving palliative care.  Candidates must be able to clearly evidence improvements relating to wellbeing and/or satisfaction levels. Evidence of an approach to how care delivery is designed, delivered and evaluated is essential.

The Best Fundraiser Award

This award recognises an outstanding individual who has a proven track record in effective and creative fundraising. Candidates should be able to evidence the amount raised, community engagement and the impact the funds have on the cause.

This award recognises organisations who provide compassionate and effective support to palliative care professionals and people facing loss. A culture of wellbeing and evidence of workforce training, advice and resources in this area should be demonstrated.

This special award recognises volunteers who demonstrate commitment, empathy, compassion and selflessness.  A positive impact for the people in receipt of care, families and staff, together with a contribution to the mission and values of the organisation should be evident.

This Award recognises teams that demonstrate exceptional multi-level collaboration across the health and social care sectors, creating lasting and sustainable impact beyond individual organisations. Entrants must showcase their ability to drive meaningful partnerships that result in measurable improvements in end-of-life care, benefiting individuals and communities. Successful applicants will highlight the broader, systemic impact of their collaborative efforts, with a focus on sustainability and the long-term enhancement of care quality during this critical stage of life.

The Palliative Care Worker Award

This award celebrates the efforts of care workers in delivering outstanding end of life care. Recognising excellence in hospice, residential/nursing home or a community setting, the judges are looking for care professionals who are dedicated to providing holistic and person centred care, an ability to effectively manage pain control and expertise in addressing the physical, emotional and spiritual needs of the individual in receipt of care and their families.  Communication, teamwork and a commitment to improving the quality of life for the individual in their care, should also be evident.

The Palliative Care Team Award

This award celebrates outstanding team work in the support of people facing the end of life in a social care or hospice setting. Recognising exceptional teams who have made a real impact in improving the overall quality of life for individuals facing serious illness. Teams should be able to demonstrate collaboration between different stakeholders; acute hospitals, community health professionals, care providers and hospices, and adopt an interdisciplinary approach to care. Great communication skills, an ability to effectively address the physical, emotional and spiritual needs of people in receipt of care and an ability to work together and support one another should also be evident.

The Diversity and Inclusion Award

This award celebrates a commitment to promoting diversity and inclusivity among staff, individuals receiving care and their families. Recognising initiatives that have successfully pushed boundaries and addressed and overcome obstacles in providing palliative care. Efforts to ensure equitable access to care, cultural sensitivity and fostering an environment that respects and values individual differences should be evident.  Initiatives to address disparities in access to care in marginalised members of our community as well as a commitment to diversity training and cultural competency  should also be apparent.

This award recognises an individual or organisation whose campaign has raised the profile of end of life care and made a real difference to the quality of care provision in this area.  Evidence of outcomes, value for money, innovation, sustainability and best practice is essential.

This award celebrates excellence in palliative nursing in a social care or hospice setting. Recognising a highly motivated individual who is passionate about providing holistic person centred end of life care.  Pain management, symptom control, communication with patients and their families and ongoing professional development should be evident.

This award celebrates excellence and innovation in training and development in palliative care. Recognising values based recruitment strategies and practices for attracting, hiring and retaining compassionate staff.  Initiatives to promote employee engagement, professional development opportunities and a supportive work culture that contribute to high retention rates and staff satisfaction must also be evident.

This award celebrates exceptional individuals who have made  significant contributions to the area of palliative and end of life care over a number of years. The award aims to recognise a real-world difference made by an outstanding individual who has revolutionised palliative care.

The Integrated Care Systems Awards

This Award recognises delivery of significant improvements and inspirational care for older people nearing the end of life across the wider ICB population. Candidates must demonstrate practical whole-system joined-up working, leading to tangible improvements in care for people nearing the end of life care. This includes affirming the role of generalist frontline teams in any setting,  excellent collaboration across health and care,  use of digital or  other  innovations , and recognising that  ‘end of life care is everyone’s business.
